Service Description: DSO Central West Region Halton Office is the point of access for Ministry of Community and Social Services funded adult developmental services and supports.

The Halton office is the contact point for families/individuals who are interested in accessing:
  • residential services and supports
  • caregiver respite
  • community participation supports (with social and recreational activities, work activities and volunteer activities, including Passport)
  • specialized services, such as service coordination or clinical services
  • person-directed planning supports

Application: Documents needed to complete an application:
Document confirming age:
* copy of a birth or baptismal certificate
* passport
* drivers licence

Proof of Canadian citizenship, landed immigrant status or permission to stay by Citizenship and Immigration Canada:
* copy of passport
* naturalization certificate
* immigration documents
* Minister’s permit

Documents confirming Ontario residency include but are not limited to:
* rental or lease agreement
* statement of direct deposit for Ontario Disability Support Program
* employer record (pay stub or letter from employer on company letterhead)
* mailed bank account statements (not automated teller receipts or bank books)
* utility bill
Eligibility - Population(s) Served: individual must have a documented developmental disability (documentation accepted is a psychological assessment) and meet age and residency requirements
Age: 18.00 years and older
Languages: English ; French ; Interpretive Services
